If in the last two weeks you have been in Iran, Italy in a place designated as a lockdown area, South Korea in a place designated as a lockdown area, or Hubei province China, do not come into the surgery. Even if you have no symptoms, stay home, away from others and call NHS 111 for advice.
If since 19th February you have been in any other part of northern Italy, any other part of Korea, or any other part of China or any of these countries: Vietnam, Cambodia, Laos, Myanmar, Thailand, Japan, Hong Kong, Taiwan, Singapore, Malaysia, Macau – and if you have got any cough, fever, or shortness of breath – do not come into the surgery, call NHS 111 and tell them about your travel and illness.

The NHS Friends and Family Test (FFT) was introduced in 2014 to help service providers and commissioners understand whether their patients are happy with the service provided, or where improvements are needed. It is a quick and anonymous way to give your views after receiving care or treatment across the NHS.

From 1st April 2015 all current and newly registered patients will be allocated to a named accountable GP. This GP is responsible for patients’ overall care at The Practice. Should you wish to establish who your present accountable GP is then please contact the Health Centre for further information.
If you have a preference as to whom your accountable GP should be, the practice will make reasonable efforts to accommodate this request.
